Job Description:
The Triage Integrated Behavioral Health Clinician provides float
coverage and crisis triage across all primary care teams at PCHC.
This person works directly with the call center, front desk staff,
integrated care team and medical care teams as the first point of
contact when a behavioral health emergency or crisis presents for
any PCHC patient. This person will assess individuals with
behavioral health and /or substance abuse issues, and provide
appropriate triage and brief treatment services. This individual
will also have a float capacity across all PCHC clinics to provide
coverage for IBH clinicians who are out for any extended
period.Essential Duties & Responsibilities:--- Maintain an active
presence on primary care clinical teams. Participate in daily
medical team huddles, medical staff meetings, multidisciplinary
teams, and individual supervision sessions with the director---
When providing float coverage consult with primary care providers
in real time to assist patients with psychosocial issues. This is
frequently done as a "warm hand-off" during hours of clinical
operation. Informal consultation and referrals from the primary
care providers happen at other times as well--- Triage both
scheduled and immediate referrals from the call center, front desk
staff, and medical teams, treating some with brief treatment and
referring others to more appropriate resources.--- Provide brief
treatment interventions for mild to moderate mental health issues
as well as crisis intervention across the lifespan inclusive of
pediatric to geriatric with comfort and competency working with
individuals, couples, and/or family members. The behavioral health
clinician should be competent in brief assessment, crisis
management, triage, diagnosis, as well as mental health / substance
abuse treatment within a brief solution-focused model. The care
model focuses on helping patients achieve practical goals with an
average of 3-6 visits. Functional schedule based upon 30-minute
appointment slots in a primary care exam room at identified site or
virtual sessions conducted remotely as warranted.--- Develop and
maintain evidence-based workflows and procedures to support the
effective implementation of behavioral health triage within
integrated behavioral health care. --- Knowledge of and ability to
apply harm reduction, build rapport quickly with patients, and
maintain a calm demeanor during stressful situations.--- Prepare
and submit all electronic and paper documentation in a complete and
timely manner. All progress will be fully completed and signed
within 72 hours of the encounter in the electronic health
record.--- Demonstrates customer-centered approach to treatment, as
measured by respectful interactions with patients and their
families, high patient satisfaction, and lack of patient
complaints--- Places high value on the treatment team, as measured
by willingness to meet regularly, work collaboratively, and
demonstrate flexibility when consulting with medical providers in
the consultation and treatment of new patients, whether they are
scheduled or referred as "warm hand-offs" or unscheduled
emergencies.--- Demonstrates a thorough understanding of the DSM
V-TR, as measured by thoroughness and accuracy of diagnostic
formulations brought for review to the multidisciplinary team.---
Shows the ability to obtain a complete history and full
conceptualization of the patient's problem within the time-limited
session(s), as measured by the quality of the written assessment
and the timely case presentation to the multidisciplinary team---
Demonstrates a thorough knowledge of effective treatment protocols
for crisis and brief solution-focused interventions and necessary
cultural competencies for treating children and adults with mental
health/substance abuse issues, as well as chronic disease lifestyle
management.--- Adapts therapeutic strategies to individual
characteristics of the patient, including but not limited to
disability, gender, sexual orientation, developmental level,
culture, ethnicity, age, health status, as measured by return
visits and client satisfaction--- Coordinated Care/Patient Centered
Medical Care Management: Works with all members of the Health Care
Team towards achieving coordinated patient centered care.
Demonstrates knowledge of the goals and objectives of PCHC's
patient care management plan. Effectively able to perform and
communicate individual task within role in the process.--- Support
the organization's mission, vision and values as it relates to
customer satisfaction in all interactions with patients, visitors,
providers and staff with an understanding and sensitivity to age,
spiritual and cultural diversity.--- Demonstrates respect, dignity,
understanding and sensitivity to individual's concerns---

Education, Experience, Skills, Qualifications:--- Masters or
Doctorate in social work or counseling from an accredited college
or university--- Licensed independent clinical social worker in the
state of Rhode Island (LICSW) or Licensed Mental Health Clinician
(LMHC) with a minimum of 2 years of experience post-licensure ---
One (1) year of crisis intervention or behavioral health telephone
triage experience--- Preferred previous experience or internship in
a medical setting which supports a primary care integration model
or experience working in emergency room or crisis centers---
Commitment to work in community health with the challenges of a
high risk, and often uninsured and diverse patient population
required.--- CPR, certification required.--- Ability to develop
effective interpersonal relationships with the health care team,
providers as well as call center and front desk staff.--- Ability
to perform duties and responsibilities across life cycle (infants,
children, adolescents, adults, prenatal and geriatric) required---
Bicultural (Spanish) skills and bilingual experience preferred---
Experience in providing managed care patient services. Commitment
to work in community health with the challenges of a high risk, and
often uninsured and diverse patient population helpful--- Proven
writing skills to document in medical record and submit
documentation--- Strong/demonstrated communication skills, written
and verbal, required--- Ability to multi-task, and manage
concurrent situations--- Computer proficiency in Microsoft Office
required--- Previous experience documenting within EHR helpful---
Knowledge of HIPAA regulations. Knowledge and experience in
information privacy/security laws, access and release of
information.--- Ability to successfully complete and pass
background/lifelinks--- Access to reliable transportation.Essential
